you have a Medical Director change, please
complete the following four (4) steps.
1.
Provide a copy of the institution's Letter of Appointment and Acceptance.
This letter must contain the following four (4) items:

Signed by the President/CEO

Position Title
-
The effective (start)
date
-
Signed by the individual accepting
the position
2.
Provide a copy of the Medical Director's CV showing:
formal education, professional licenses/board certifications, and related
experience
3.
Complete this "Personnel
Change"
form and email it to office@jrccvt.org.
4. Please mail all
three (3) completed documents to:
