Instructions for Program Director Personnel Change
 

 

If you have a Program Director change, please complete the following four (4) steps.

 

1.    Provide a copy of the institution's Letter of Appointment and Acceptance

       This letter must contain the following four (4) items:  

 

  • Signed by the President/CEO

  • Position Title

  • The effective (start) date

  • Signed by the individual accepting the position

     

2.    Provide a copy of the Program Director's CV showing:

          formal education, professional credentials, and related experience.

 

3.    Complete this "Personnel Change" form and email it to office@jrccvt.org.

 

4.    Please mail all three (3) completed documents to:
